Access to Medical Records
Patients can request to access their medical records under the General Data Protection Regulations (GDPR). To request a copy of your medical records please carefully read our Access to Medical Records Leaflet and complete an Application to Access Medical Records Form which should be returned to the Practice Manager.
Under GDPR, the practice aims to respond to all requests within one month and requests made under the Subject Access Request Policy will be free of charge. However, if a request is deemed to be unfounded, excessive or repetitive, a reasonable fee may be charged. A reasonable fee may also be charged when requests for additional copies of the same information are made.
Should you identify an inaccuracy in your medical record you have a right under GDPR to request for it to be corrected. To do this please complete an Amendment to Health Records Form and return it to the Practice Manager. It is important to note that in terms of medical records, factually accurate information is unlikely to be erased or amended, however in the event of a patient disagreeing with the practice an addendum will be added to the record to state this.
